Community Health Investigation Notebook

A student research notebook documenting the investigation from question formation through data collection, interview notes, physiology notes, analysis, and personal conclusions. It proves each student’s individual mastery of the research process and content understanding.

Community Health Research Symposium Presentation and Report

A team report and formal presentation that synthesize shared survey and interview evidence into findings, visualizations, conclusions, limitations, and recommendations for community action. It demonstrates collaborative application and depends on each member’s documented contribution.
